Daniel Farris Woodhurst filed for a pension on December 9th 1911, when he was living at Richmond in Wayne County, Indiana. The material below is a transcription of the original declaration form from the National Archives.

On this 9 day of December A.D. one thousand nine hundred and Eleven personally appeared before me, a NOTARY PUBLIC within and for the county and State aforesaid, Daniel F. Woodhurst, who, being duly sworn according to law, declares that he is 70 years of age, and a resident of Richmond county of Wayne, State of Indiana; and that he is the identical person who was ENROLLED at Hamilton, Ohio under the name of Daniel F. Woodhurst, on the 26th day of August, 1861, as a private, in Company-C-35th. Regiment Ohio Volunteer Infantry in the service of the United States, in the Civil War, and was HONOURABLY DISCHARGED at Chatanooga Tennessee, on the 23 day of September 1864.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 73/4 inches; complexion, light; color of eyes, light blue; color of hair, light; that his occupation was farmer; that he was born December 9th, 1841 at Hartford, Connecticut.
That his several places of residence since leaving the service have been as follows: Jacksonboro Ohio; Franklin Ohio 1867; Greenville Ohio 1869; back to Jacksonboro Ohio 1872; Moved to Richmond Indiana in 1877. [Probably a misrecording of 1887 - other evidence shows that the latter move occurred in the period 1884-88.]
That he is a pensioner. That he has heretofore applied for pension 765800.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.
That his post-office address is 413 Chestnut Street, Richmond, County of Wayne, State of Indiana.

Also personally appeared Lillie Martin, residing in Richmond, Ind. and Ada Bellinger, residing in Richmond, Indiana, persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say that they were present and saw Daniel F. Woodhurst, the claimant, sign his name (or make his mark) to the foregoing declaration; that they have every reason to believe, from the appearance of the claimant and their acquaintance with him of One years and One years, respectively, that he is the identical person he represents himself to be, and that they have no interest in the prosecution of this claim.
